Virginia has also received numerous calls asking for guidance about deferring TB testing upon hire.  We have not recommended that this testing be deferred.  We have recommended that our local health departments, who are overwhelmed with COVID-19 response activities, stop providing this service for the time being and instruct inquirers to seek the service elsewhere.  

One recommendation we have made is that TB testing upon hire be done with a single IGRA instead of two step TST testing.

I would be inclined not to allow that process since there will likely be even more undiagnosed patients coming through the system during this timeframe so it is essential that we know the baseline coming in. Also, per the MMWR and discussions, the cases have been identified at time of hire rather than on the annual screening which is one of the reasons for the change in recommendations. I would be apprehensive and not supportive to allow this practice (not testing at the time of hire).

  • Right client
  • Right route
  • Right drug
  • Right dose
  • Right time

A sixth right often mentioned, but not part of the original five, is the right documentation. Don't forget to administer the drug to the client first, and then document. Documenting beforehand is considered falsification of documentation and is a violation of the nurse practice act.

It is a crazy time for all of us now, and I appreciate any comments or help in advance.

 

I received a call regarding questions on deferring TB testing on new hires, specifically healthcare workers since most are having issues accessing testing facilities or having to address the rapid hire to replace those out ill.  Our state has moved to essential services only from providers and most are not seeing patients for TB testing for work/administration processes. Has anyone else encountered this question. I understand that some agencies here have deferred fingerprinting until this is all over.

 

I am inclined to defer testing but require symptoms screening and insist that testing be completed within 15 days of public health order lift. Any thoughts? I haven’t seen anything come out of NIOSH/CDC/OSHA regarding deferring.

 

Again, any insights would be appreciated.
