Dear TB Nurse Colleagues,

Thank you to those who have responded regarding usefulness of the MDR Side Effect Tool and to those of you who have begun pilot testing in your area! The input will be very valuable in making the tool practical for nurses in the US.

Please note, for those of you who are pilot testing or have shared with other nurses in your region, we now have the evaluation form on survey monkey. If possible, please use this to complete your evaluation of the tool.

And, if you haven’t begun pilot testing, there is still time! We are aiming to have feedback by August 31^st

Below are the instructions for pilot testing:

· Review the entire tool (attached)

· Refer to the tool if a patient presents with one of the listed symptoms. Review the suggested nursing assessments and interventions and make note on the evaluation form if an assessment and intervention was used. (Note: only perform listed nursing assessment and interventions if appropriate in your setting and within your regular scope of practice.)

· Use and refer to the tool for approximately 1-3 weeks

· Complete the evaluation form by August 31, 2017 at https://www.surveymonkey.com/r/DCYZ8PT
