I have worked in TB for about 6 years. The biggest stressors in this position is training and staffing. We are set up where TB Control refers our case management out to a staff of public health nurses in another department. This creates issues because they have so many other responsibilities and we are located all over the city so it is very difficult to monitor their care. We meet monthly to discuss with our TB Medical Director, but we frequently find errors/issues in case management after it is too late.

 

I get the most joy from helping clients. We run a free TB clinic. Most of our clients face major social barriers, I enjoy helping to overcome or address those barriers. I also enjoy being able to work with people from all over the globe on a local level.

I am into my sixth year of working in TB. I have found my time working with each and every client very rewarding. I think we all deal with staffing issues which makes our jobs a bit more difficult but my most difficult situations are the limitations of our outreach to our clients. I strive each time we get another client to do my best with them. What makes me happiest, is a happy well client who is able to return a normal pre-TB life.

1. How long have you worked in TB? I have worked in public health for almost 25 years. I have worked in TB Program for almost 20 years. I am now a nurse consultant and have worked at the state level since 2002. I worked as a front line TB nurse for 9 years.
2. What is your biggest stressor working in TB? (Ex. no staff, no training) Our biggest challenge is staffing and training. We are short staff at the county and state level so this puts a lot of stress on the TB program. It is difficult to get adequate training done for new staff since there are shortage and the counties need to take care of the the patients. The high acuity level of our patients is also a stressor. We do have social workers that are assigned to the TB program to help assist our patients.
3. What gives you the most joy from working in TB? The things that gives me and all the staff joy is the successful completion of treatment and to see patients go from very sick to being well.

I have worked in TB control for a little over 4 years after spending 30+ years in hospital nursing in Pediatrics.I was a little anxious as I have never worked with the adult population but was looking forward to no weekends or holidays having to work. Besides lack of adequate staffing my biggest stressor is having patients so acutely ill,and not hospitalized-the lack of back-up emergency equipment and the lack of other 'eyes' to assist in assessments. Also their social needs are so great I sometimes feel like a social worker trying to get them the assistance they need. I never would have guessed how attached to these clients I become, we become part of their extended families and the I feel as if I am really making a difference in their lives. Seeing them go from so acutely ill to returning to work is very satisfying.

2 years.MDR casesHaving an advanced TB patient with MDR TB almost die several times and then actually survive. The most difficult cases can be the most rewarding.

1.       12 years

2.       Currently being  furloughed one day per pay period and an increase in cost of benefits ($10,000+ pay cut) through June 30th 2018. Also working with ¾ of the staff we had 8 years ago.

3.       Helping others (nurses, doctors and patients) when they are lost or scared.

2. What is your biggest stressor working in TB? (Ex. no staff, no training)   Lack of staff and no one or current policy/procedure to look to for direction on how things were done previously.  Unfamiliar with the EHR
3. What gives you the most joy from working in TB?  Helping patients progress, seeing them get well and complete treatment. Assisting those who are in need of other assistance.

I have been a TB Nurse Consultant for almost 13 months. My background in TB comes from working as a PHN at a local health dept. and as an Employee Health Nurse. My biggest stressor was working in a program where everyone is new and the other TB Nurse Consultant had just left to work overseas for a year, with no replacement. Now my biggest stressor is not enough training and not having enough time to do the readings and webinars that would help me. I like working as a TB Nurse Consultant, I find it interesting and challenging, no two cases are ever the same. I like educating and supporting the PHNs at LHDs.

I have worked in TB for 7 years.

A common stressor is the lack of staff/resources in rural areas/reservation to provide DOT.

Working in TB is very rewarding. I love working directly with the clients and developing those relationships. Most of the clients are very appreciative and frequently thank me for saving their life. There is always something to learn in TB. J

That has lead me on a fact finding mission and I would like to pose three questions to all of the NTNC members out there.

1. How long have you worked in TB?  A little over a year.
2. What is your biggest stressor working in TB? (Ex. no staff, no training)   Lack of staff and no one or current policy/procedure to look to for direction on how things were done previously.  Unfamiliar with the EHR
3. What gives you the most joy from working in TB?  Helping patients progress, seeing them get well and complete treatment. Assisting those who are in need of other assistance.

I have been a TB Nurse Consultant for almost 13 months. My background in TB comes from working as a PHN at a local health dept. and as an Employee Health Nurse. My biggest stressor was working in a program where everyone is new and the other TB Nurse Consultant had just left to work overseas for a year, with no replacement. Now my biggest stressor is not enough training and not having enough time to do the readings and webinars that would help me. I like working as a TB Nurse Consultant, I find it interesting and challenging, no two cases are ever the same. I like educating and supporting the PHNs at LHDs.

Hope everyone is looking forward to a well deserved relaxing weekend.

Recent,y I have been having discussions with many nurses across the TB world and many seem stretched to the limit with little recognition of all that goes into managing TB , yet we are very passionate about our work and love what we do in this unique niche in nursing.
