Usually we have just done that with phone calls. We might send the interjurisdictional form about the case but we let them know about the exposure to the citizens.

Hello TB Nurses!

Quick question: Has anyone ever put together a Contact Investigation Request Form for jurisdictions to use when requesting a CI from an outside jurisdiction?

Any help is greatly appreciated.
